Use this profile for an allergy or intolerance that dental care documents and maintains locally. Information retrieved from Kjernejournal remains a separate source; a local record should represent a clinical assertion, not a copy made merely because external information was viewed.

Map the assertion

A recorded allergy has two different status questions: whether it is clinically active and whether the assertion is confirmed, provisional or entered in error. Preserve both meanings from the source. The agent belongs in the allergy record; do not substitute a medication order or a Condition for it. The clinician who records the assertion and the person who provided it can differ; use recorder and asserter accordingly when the source distinguishes them.

For example, a Kjernejournal alert alone stays external. If a clinician separately assesses and records a latex allergy in the dental journal, that local assertion can use this profile. The current profile does not carry structured historical reactions, and the absence of a local allergy record is not evidence that the patient has no allergies.
