The Integrated Assessment Record (IAR) solution enables health information custodians (HICs) to submit completed assessments to the IAR repository.
This FHIR Implementation Guide (IG) is intended to define the standardized data formats, structures, and terminologies required for these exchanges. For this release, the scope is limited to the submission of completed assessments only.
This IAR FHIR IG specification version focuses on a stable set of FHIR resources. These resources represent the finalized set for this version and cover the submission of completed assessments only.
Example assessment forms are provided for illustrative purposes only to demonstrate how assessment data capture may be structured in alignment with the HL7® FHIR® Structured Data Capture (SDC) and the Questionnaire resource. These examples are not normative and/or a required technical artifacts for implementors to use.

Ontario Health will be establishing the Ontario Integrated Assessment Record (IAR) as an interoperability specification under Ontario’s Digital Health Information Exchange (DHIEX) framework.
The following information is provided in accordance with O. Reg. 329/04 under Personal Health Information Protection Act (PHIPA), 2004, section 28 Application of Specifications.
