2.2.1. IAR Conceptual Data Model

The Conceptual Model below is a high-level representation showing the entities, their relationships, and how IAR uses FHIR resources to exchange assessment data.

2.2.1.1. Assessment

The information model for an assessment submission is represented as a FHIR message Bundle that includes the MessageHeader, Patient, Encounter, Organization, Location, and one or more QuestionnaireResponse resources. The MessageHeader describes the event represented by the message. The Encounter links the Patient and the Organization involved in the assessment, and each QuestionnaireResponse is associated with both the Patient and the Encounter. The Questionnaire itself is not included in the bundle and is instead referenced externally. Account, Coverage, and Location details are included within the Encounter as contained resources.
