4.3. Extensions

This page lists all of the Extensions defined in this implementation guide. There may be additional extensions used from external sources such as the HL7 Extension Registry which are referenced in this guide.

ExtensionDescription
Extension: Birth Sex

A code classifying the person's sex assigned at birth. Replicates the birthsex extension in US-Core-R4 for interoperability.

A code classifying the person's ethnic group or ethnicity as the category of people who identify with each other, usually on the basis of a presumed common genealogy or ancestry or on other similarities. This should not be confused with citizenship or nationality.

Indigenous Identity

A code classifying a person's self-identification as either First Nations, Inuk/Inuit and/or Métis.

Individual Gender Identity

An individual's personal sense of being a man, woman, boy, girl, nonbinary, or something else. This represents an individual’s identity, ascertained by asking them what that identity is. In the case where the gender identity is communicated by a third party, for example, if a spouse indicates the gender identity of their partner on an intake form, a Provenance resource can be used with a Provenance.target referring to the Patient, with a targetElement extension identifying the gender identity extension as the target element within the Patient resource. When exchanging this concept, refer to the guidance in the Gender Harmony Implementation Guide.

Questionnaire Name

An extension that records the code and name of the questionnaire used in a QuestionnaireResponse.
