2.1. Business Model

2.1.1. Context

This document outlines the business context for the Integrated Assessment Record (IAR) solution. The goal is to enable Health Information Custodians (HICs) in the community care sector to electronically submit completed assessments to the IAR Clinical Data Repository (i.e. IAR CDR) and, in future releases, to access and view completed assessments.

2.1.2. Model

2.1.2.1. Conceptual Architecture

This diagram illustrates the high-level process flow for submitting a completed IAR assessment form, starting with Data Contributors (i.e. HIC applications) and ending in the IAR CDR.

2.1.2.2. Systems

The sections below describe the interaction model between Data Contributors and the IAR CDR, with diagrams illustrating the high-level interaction flow.

The systems in this state include:

  • Data Contributor (HIC Apps): The source system submitting the assessment form data.
  • FHIR Server (IAR CDR): The initial receiving endpoint for the FHIR message which acts as pass-through only.
  • Internal Data Exchange (multiple processes): Represents several internal operations.
