Use Case - Update Previously Submitted Patient Summary

This use case provides the capability for submission of updated information from an EMR/POS, when a patient summary has previously been submitted from a given patient’s chart.

Pre-Condition 1: HCP is registered for submission of provincial patient summaries.

Pre-Condition 2: A standardized patient summary has previously been submitted to the provincial Hub service from the patient’s chart.

Pre-Condition 3: The patient’s chart is managed by the same healthcare organization as the previous submission.

Basic Flow: Compile and Submit Updated Patient Summary

The Basic Flow is initiated automatically as updates are applied on a chart where a patient summary has been previously submitted.

1. Update Patient Chart for any Patient Summary Data Category(ies)

HCP adds, edits or removes information within their patient chart, associated to any of the standard patient summary data categories.

2. Generate Updated Patient Summary

POS applies the updates to the set of curated data from the previous patient summary submission, once the chart updates are complete (finalized).

  • Business Rule - Apply Updates to Curated Patient Summary: New and updated information is incorporated into a previously curated patient summary as follows:
    • Additional data is automatically incorporated under any patient summary category.
    • Updates to previously excluded data (as curated by the authoring provider) remain excluded, unless / until the authoring provider instructs the POS to now include.
    • All previously curated notes are retained and included in the updated patient summary extract.
  • Business Rule – Allow Curation of Updated Patient Summary: The POS must provide an option for an authoring provider to choose to curate patient summary content before updates are automatically submitted. This includes some mechanism to inform the charting provider that an updated patient summary has been generated and is ready for submission.

3. Submit Updated Patient Summary

POS submits the assembled patient summary data to the provincial service.

  • Business Rule – Retain Updated Patient Summary Data: The assembled patient summary is retained as an updated record within the patient’s chart.
  • Business Rule – Patient Summary Update Submission Metadata: The following metadata is included for an updated patient summary:
    • Patient Summary Identification: The unique identifier is retained from the original patient summary submission (when generated from the same patient POS chart, for the same submitting organization and author)
    • Submission date and time
    • Submission type – patient summary (with related bundle information)
    • Submission status – ‘amended’

Alternate Flow: Curate Updated Patient Summary Before Submission

This Alternate Flow is initiated from the Generate Updated Patient Summary step of the Basic Flow, when an authoring provider chooses to view and curate patient summary updates before submission to the provincial service.

1. Present Generated Patient Summary with Updated Chart Information

POS presents the generated patient summary with chart additions and/or changes incorporated into the previously curated patient summary.

2. Re-Assemble Patient Summary with Curated Information

HCP curates the updated patient summary as allowed for new patient summary curation, and instructs the EMR/POS to finalize and submit.

3. Return

Return to the Submit Updated Patient Summary step of the Basic Flow.
