NPCR_Gleason_Score_Systematic_Biopsy_Observation
Gleason score observation for prostate tissue obtained via systematic biopsy. Part of the NPCR Gleason score observation family, consisting of one Summary-level profile (NPCRGleasonScoreObservation) and four source-specific profiles.

Systematic biopsy uses a standardized multi-sector sampling convention covering the entire prostate, yielding a single Gleason assessment per biopsy session. Unlike targeted biopsy, no reporting convention choice is required (D_RiktGleasTyp is not applicable), so this profile does not include the NPCRGleasonDefinitionType extension.
The Summary Gleason Observation (NPCRGleasonScoreObservation) aggregates this and other source-specific observations using FHIR Observation.derivedFrom references. Per NPCR INCA logic, systematic biopsy is the first-priority source for derivation when D_Vavn=1 (before Oklar and Rikt).
Terminology binding — documentation gap (Observation.code): NPCR Informationsspecifikation §7 does not provide a section for Gleason score identification. Observation.code is Must Support but without fixed value or terminology binding, consistent with other NPCR observation profiles. Resource identification relies on meta.profile.
