Finnish PHR QuestionnaireResponse Profile
Finnish PHR profile for FHIR® QuestionnaireResponse. A QuestionnaireResponse provides answers to a set of questions filled when responding to a questionnaire.
This profile only defines the structures used in questionnaire responses, not the actual content of the questionnaire response. The actual content of the QuestionnaireResponse is based on the Questionnaire resource it refers to. All the QuestionnaireResponses stored in Kanta PHR must follow these structures and profiling done to the elements of the resource.

| Questionnaire answered | questionnaire | A reference to the answered Questionnaire. The reference should be to the url of a Questionnaire resource stored in Kanta PHR. |
| Status | status | The status of the questionnaire response indicating for example whether the answering has been completed or still in progress. The code for the status SHALL be from the FHIR Questionnaire Response Status ValueSet, however, the code 'entered-in-error' is not allowed in Kanta PHR. |
| Subject of the questionnaire response | subject | The patient/citizen the answers apply to/are about. The reference SHALL be to a Patient resource stored in Kanta PHR (that conforms to the Finnish PHR Patient profile). |
| Date the answers were gathered | authored | The date and/or time that this set of answers were last changed. |
| Who answered the questions | source | This may be the same as the subject, or this may be another person for example when someone is using Kanta PHR on behalf of the subject. The reference SHALL be to a Patient resource stored in Kanta PHR (that conforms to the Finnish PHR Patient profile). |

Kanta PHR platform will fill in this data automatically to each stored resource (if the application has entered some data in this extension, it will be overwitten). |
Finnish PHR QuestionnaireResponse Narrative guide
Narrative of the Questionnaire Response Profile must include either all the questions and answers of the Questionnaire or a summary of the essential information.
