Finnish PHR SelfCarePlan profile
Finnish PHR profile for FHIR® CarePlan to be used with self care plans. NB! This profile is used with self care plans that are generated on the basis of a decision support system.
NB! At the moment SelfCarePlan's narrative includes the questions and answers from the Questionnaire and QuestionnaireResponse that the plan is based on.

Mandatory elements for Finnish PHR Self Care Plan

| Language | language | Language used in the self care plan stored in Kanta PHR. Language SHALL be chosen from the FHIR languages ValueSet. |
| Narrative | text | A text summary of the self care plan. SHALL include all content for a human to understand the essential clinical and business information of the self care plan. Narrative also includes the questions and responses from the Questionnaire and QuestionnaireResponse on which the plan is based on. |
| Unique identifier | identifier | At least one globally unique identifier has to be provided for the observation, in addition other identifiers may be given as well. This identifier may be used to uniquely identify the observation if the application doesn't receive an id for the observation provided by Kanta PHR. |
| Status | status | Indicates whether the plan is currently being acted upon, represents future intentions or is now a historical record. The code for status SHALL be from the FHIR Care Plan Status ValueSet, and only the following codes are allowed in Kanta PHR: completed. |
| Intent of the plan | intent | Indicates the level of authority/intentionality associated with the care plan. The code SHALL always be 'plan' from the FHIR Care Plan Intent ValueSet. |
| Category of the plan | category | Identifies what kind of plan it is. The code SHALL be from the Finnish PHR CarePlan Category ValueSet. |
| Title | title | A human friendly name of the self care plan. |
| Description | description | A description of the scope and nature of the plan. Provides more detail than conveyed by category. |
| Who the plan is for | subject | Identifies the patient whose intended care is described by the plan. The reference SHALL be to a Patient resource stored in Kanta PHR (that conforms to the Finnish PHR Patient profile). |
| Person who is responsible for the contents of the plan | author | This may be the same as the subject, or this may be another person for example when someone is using Kanta PHR on behalf of the subject. The reference SHALL be to a Patient resource stored in Kanta PHR (that conforms to the Finnish PHR Patient profile). |
