Finnish PHR Questionnaire Profiles

The Questionaire and Questionnaire response profiles defined in this Finnish PHR Implementation Guide constrain and extend FHIR® Questionnaire and FHIR® QuestionnaireResponse resources.

The aim is to have a single Questionnaire profile and a single QuestionnaireResponse profile in the Finnish PHR, that all the Questionnaires and QuestionnaireResponses stored in the Finnish PHR will conform to.

Finnish PHR Questionnaire Profile: The Questionnaire is an organized collection of questions intended to solicit information from patients.

Finnish PHR QuestionnaireResponse Profile: The Questionnaire Response provides answers to a set of questions filled when responding to a questionnaire.
