HbA1c target

The target ranges in FHIRSTACK is stored as separate Goal resources which are referenced in a target ranges careplan. Further, the target ranges careplan points to a master careplan for the patient. The same hierarchy is to be followed to maintain uniformity across integrations. Also, if there will be more targets other than HbA1c sent in future, the target ranges careplan will help consolidating all the ranges for a patient.

Since there are references to other resources and NextMed uses the FHIR API for creating individual resources, the following resource need to be created in this order. The "id" of the created resource needs to be used in the references of the resources created next.

1. Master CarePlan

Master CarePlan for the patient. There will be one master careplan for a patient.

2. Goal

Goal resource containing the HbA1c target range. Typically, there can be multiple goal resources (like HbA1c target, weight, BP, BG range etc)

3. Target ranges CarePlan

Target ranges careplan that references the Goal resource and the master careplan. There will be one target ranges careplan for a patient.

Drug Class

This is an example for Current Drug Class Name.

Note: Drug class can be retrieved by using an FHIR API Substance endpoint. In order to obtain drug class, the following steps need to be followed.

  • NexMed application has to retrieve all the current MedicationRequests
  • MedicationRequest has medicationReference element where substance code can be obtained
     "medicationReference": {
     	"reference": "Medication/medicationB02BC05" //reference to the medication resource
     }

  • NexMed application can send GET request with Substance code (5 chararters of ATC Code -> ex B02BC)
  • Response contain substance family resource for the specific MedicationRequest

Command 'json' could not render: File not found for 'subject=example-therapies-substance-class'
