Use Cases

The use cases below provide basic illustrations of how actors interact with the Mental Health and Addictions Provincial Data Set (MHA PDS) Repository to contribute client, health information custodian, and program information.

Actors

Actor Type Description
Client Human Subject who is receiving care at a health information custodian organization, and for whom data is being collected.
Health Information Custodian (HIC) Organization The organization where the client is receiving care.
Point of Service (PoS) System System Point of Service (PoS) Systems is an electronic product, service or system that uses electronic means to collect, use, modify, disclose, retain or dispose of PHI, and that is selected, developed or used by a HIC. Examples include but are not limited to: an electronic medical record (EMR), a hospital information system (HIS), and clinical viewer.
MHA PDS Repository System The recipient of assembled client data.
Authorized PoS User Human The person who enters client information into the PoS system.

UC-01 - Initial Submission of Client Data to the MHA-PDS

Summary

The initial submission to the MHA PDS repository will include all active clients, and clients that actively referred to programs or are currently enrolled into programs provided by the Health Service Provider (HSP). The initial submission is required to be sent to Ontario Health one time as this is the first scheduled FHIR submission from the HSP. The initial submission should also include historical records for all clients active within one full prior fiscal year, plus the data for the current fiscal year up to the date of onboarding.

Preconditions

  • A Client is assessed or receives care services from a HIC location site virtually or in-person
  • MHA PDS HL7 FHIR IG requirements are met to a minimum

Primary Flow

  • The HIC has successfully completed the onboarding process and is ready to Go-Live.
  • Test data is submitted to the MHA PDS Repository by the PoS for data validation.
  • The test data is validated by Ontario Health to ensure the data conforms to MHA PDS HL7 FHIR IG requirements.
  • Client, Health Service, Referral, Program, Encounter, and Health Service Provider site information is assembled and summited to the MHA PDS Repository.
  • Submission status (success / reject) is sent back to the PoS system.

Post-Conditions

The initial submission package containing all active client records from the HIC will be submitted to the OH repository where it is available for authorized access.

UC-02 - Ongoing Submission of Client Data to the MHA PDS

Summary

A client visits a HIC location site (virtually or in-person), triggering client data to be entered and stored in a PoS system. Client data is collected and assembled in the PoS system. Upon collection of minimally required data elements as per the Mental Health and Addictions Provincial Data Set HL7 FHIR implementation guide, the PoS system submits the assembled data to the MHA PDS Repository.

Preconditions

  • A Client is assessed or receives care services from a HIC location site virtually or in-person.
  • MHA PDS HL7 FHIR IG requirements are met to a minimum.

Primary Flow

  • A Client arrives at the HIC location.
  • Client information is entered into the PoS system.
  • Additional client data is collected, including but not limited to client social determinants of health, episode of care, health program information, and encounter data.
  • Client data for a single client record is assembled and prepared for submission to the MHA PDS Repository.
  • Data submitted to the MHA PDS Repository by the PoS system. Each data submission will consist of a single bundle resource containing one client record.
  • Add record success/reject sent back to the PoS system.

Post-Conditions

A Client record is added to the MHA PDS Repository where it is available for authorized access.
