Registro de medicamentos que un paciente afirma estar tomando o haber tomado, o que un profesional de la salud identifica como consumo actual o previo del paciente durante un proceso de atención.
La declaración de uso de medicamentos es la manifestación, por parte del paciente o de un informante autorizado, del uso de uno o más medicamentos, o el reconocimiento por parte de un profesional de salud de la utilización de medicamentos, con el objetivo de complementar la historia clínica, identificar posibles interacciones, prevenir eventos adversos y garantizar la continuidad y seguridad de la atención.
