Medication

For the scope of M1, a dummy Medication resource is created for every Substance resource created with a reference to it.

Note: In FHIR STU3 there is no identifier element in the Medication resource. So only the internal id serves as the unique identifier. For M1, a Medication reosurce should only be created when a Substance resource is created. i.e. there will be one Medication resource for each substance resource. When we implement the next milestones, we may need to consider other elements for defining de-duplication rules.

For the scope of M3, a Medication resource is created for commercial medications and relevant existing Substance (including compound substance available in database) resource in fhirserver will be reference to it.
