Profile: Social History

Definition

Social history includes health related lifestyle factors or lifestyle observations. Currently, Smoking and Alcohol use are considered, though it may include other factors such as use of other substances, or family and living situation. The Observation Resource contains a set of proposed constraints for general use. This profile sets minimum expectations for the Observation resource to represent various observations if no other, more specific profile is applicable. An observation is a measurement or assertion about a subject (person, group, device, location, or another subject).

Context

The SHx Observation: Social History (PS-CA) profile represents the constraints applied to the Observation resource by the PS-CA project to represent a more generic social history profile used to represent health related lifestyle factors or lifestyle observations in a patient summary. Currently, there is no equivalent IPS UV profile, however there are ISO CEN IPS 17269 requirements for a more generic social history section that were used to inform this profile.

This profile is considered a draft version and is informed and created as per the requirements of the first jurisdiction (i.e. Alberta) to pilot its use in patient summary within jurisdictional context. The constraints have been compared against other Social History FHIR profiles in Canada and internationally (US Core, Cyprus Core, Malaysia Core) and share many constraints in common with those profiles but represent the minimum requirements as they are known in Canada today. This profile is expected to be refined further as more jurisdictional requirements are defined and as more is known about use in the international patient summary space.

In the PS-CA:BC Release 1, Smoking and Alcohol use are included as specific observations, with narrative to describe other observations. As clinical requirements are more defined, future releases will likely include more observations. The data source will be the Provider EMR.

Usage

The Observation Resource is used to populate entries in the Social History section of a Patient Summary.

Social History (Composition.section:sectionSocialHistory)

  • Definition: Measurements and simple assertions made about a patient, device or other subject.

Observation.value[x]:valueCodeableConcept

  • Definition: A measured amount (or an amount that can potentially be measured). Note that measured amounts include amounts that are not precisely quantified, including amounts involving arbitrary units and floating currencies. This profile imposes that the code system for units be UCUM.
  • Comments: This element is referenced from a different profile "Observation Alcohol Use (PS-CA)".

  • Comments: Must Support and Optional with multiples value. This is a Free Text string element at the Bottom, which can be utilized to include additional information.
