Initial MVP Release 1 Scope (Transactional Patient Summary)

Primary Care Provider (PCP) generates patient summary in their EMR and sends it to acute care Clinical Information System for use by providers to support an admission to acute care. Patient summaries will be saved in the sending provider's EMR, as well as in the receiving Clinical Information System.

  • Access to the patient summary is controlled (sender and clinical endpoint).
  • Each updated patient summary is stored and accessed as a unique patient summary.

Future Release Scope

Transactional Patient Summary

  • Patient admitted to hospital, PCP is notified/informed of the admission via existing notification methods; PCP creates and sends patient summary to acute clinical information system for use by providers.​​
  • Patient summaries attached as part of other Information Request such as:​​
    • PCP generates patient summary and attaches to electronic or paper documentation (e.g., eReferral, eConsult or transfer of care request) as supplemental information.
    • PCP generates patient summary and stores within EMR to support coordination of care within practice (e.g., on-call, locum coverage) as quick overview of patient information.
    • PCP generates patient summary and stores within EMR and provides to patient on patient request.

On-demand Patient Summary (Information stored and accessed as required from a repository)

  • PCP generates patient summary and sends it to the provincial repository​​
    • Patient summary may include updated information as it is received from the EMR or other data sources (i.e., point-in-time may be updated from multiple sources on an ongoing basis).
  • Admitted to Emergency Department/Acute Care​​
    • Patient admitted to hospital, and PCP notified via in-app notification that patient summary is required > PCP creates and sends patient summary to acute care site for use by providers.
  • Provider Access​​
    • A provider with authorized access to the repository can access and view the patient summary.
  • Patient Access​​
    • Future scope is for patient to have access to their patient summaries.
