2.2. Business Data

2.2.1. Conceptual Data Model

The Conceptual Model presented below is tied directly to the HL7 FHIR interface specification and uses FHIR Resources for entities for the transmission of the OCRE data. The model does not repeat the structure of the messages laid out in FHIR. Links to FHIR specs are included for entity definitions and field code values. In any difference of meaning between cited FHIR elements and this model, FHIR is the authoritative voice. This model relies on the following specifications:
  • FHIR R4.0.1 - The current official version of FHIR as of the time this implementation guide was published

2.2.1.1. Unstructured Reports (Clinical Reports)

Unstructured reports are centered around FHIR Documents – Bundles with a Composition resource along with the Patient, Encounter, Practitioner and other reference data. Unstructured reports will be supported as a Binary resource which may accept a text data, pdf, image, etc.

A data contributing system submits a Document Bundle containing the Composition resource and all other resources mentioned below.

2.2.1.2. Structured Reports

Structured reports are expressed as Compositions sections pointing to clinical FHIR resources, e.g. Observation, Condition, AllergyIntolerance, Procedure, etc. As use cases for the structured reports are not defined yet, the first version of the OCRE FHIR IG will provide some samples of Discharge Summary and Consultation Note based on the C-CDA on FHIR IG with localization. A data contributing system submits a Document Bundle containing the Composition resource and all other resources mentioned below.

2.2.1.3. Medical Imaging Reports

Medical Imaging reports are centered around DiagnosticReport resource with references to Patient, ServiceRequest, Observation, Practitioner, Encounter and other reference resources. A data contributing system submits a Transaction Bundle containing the DiagnosticReport resource and all other resources mentioned below.
