Inpatient eDischarge

The Transfer Of Care eDischarge Specification supports the following care communications:

eDischarge (inpatient discharge summary) Document – A FHIR Document containing Transfer of Care information supporting an inpatient discharge typically between an acute hospital and GP practice.

FHIR Messaging components specified within this site have been developed by NHS England and use CareConnect profiles created in collaboration with the INTEROPen community.

The INTEROPen vision is to create a library of nationally defined HL7® FHIR® resources and interaction patterns that implementers can adopt to simplify integration and interoperability within UK health and social care.

This documentation builds on work done by the Professional Record Standards Body (PRSB) in defining headings for clinical documents.


Outpatient Letter

The R4 Outpatient Letter Specification supports the following care communications:

Outpatient Letter Document – An FHIR Document containing care information between the hospital clinic and the GP and other relevant parties following a consultation in a clinic. Not adopting this message type should only be considered where a procedure is conducted in an outpatient setting that generates a structured report which is an established means of GP communication.

Mental Health eDischarge

The Transfer of Care Mental Health eDischarge Specification supports the following care communications:

Mental Health eDischarge (discharge summary to GP for mental health adult patients) Document - An FHIR Document containing Transfer of Care information supporting a discharge summary to GP for mental health adult patients. Paediatric mental health discharges are not within the scope of this use case.
