Inpatient eDischarge

FHIR resource profiles combined to support eDischarge (inpatient discharge summary) FHIR Documents.

The Bundle consists of the following FHIR Resource Profiles:

  • UKCore-Bundle - This profile defines the UK constraints and extensions on the International FHIR resource Bundle.
  • UKCore-Composition - This profile defines the UK constraints and extensions on the International FHIR resource Composition.
  • UKCore-Encounter - This profile defines the UK constraints and extensions on the International FHIR resource Encounter. The Encounter Resource represents an Encounter between a care professional and the patient (or patient’s record).
  • UKCore-List - This profile defines the UK constraints and extensions on the International FHIR resource List. It can record a snapshot of the patient's list of:
    • Allergies
    • Medications
  • UKCore-MedicationStatement - This profile defines the UK constraints and extensions on the International FHIR resource MedicationStatement. The MedicationStatement Resource is a record of a medication that is being consumed by a patient.
  • UKCore-Medication - This profile defines the UK constraints and extensions on the International FHIR resource Medication. The Medication Resource is primarily used for the identification and definition of a medication.
  • UKCore-MedicationRequest -This profile defines the UK constraints and extensions on the International FHIR resource MedicationRequest.
  • UKCore-Flag - This profile defines the UK constraints and extensions on the International FHIR resource Flag. It can carry prospective warnings of potential issues related to the patient's:
    • Allergies
    • Medications
  • UKCore-AllergyIntolerance -This profile defines the UK constraints and extensions on the International FHIR resource AllergyIntolerance. The AllergyIntolerance Resource records risk of harmful or undesirable, physiological response which is unique to an individual and associated with exposure to a substance.
  • UKCore-Condition - This profile defines the UK constraints and extensions on the International FHIR resource Condition. The Condition resource records detailed information about conditions or diagnoses recognised by a clinician.
  • UKCore-Procedure - This profile defines the UK constraints and extensions on the International FHIR resource Procedure. The Procedure resource is used to record an action that is or was performed on a patient.
  • UKCore-Observation - This profile defines the UK constraints and extensions on the International FHIR resource Observation. The Observation resource is used for tracking the current and historical observations that have been made for a patient.
  • UKCore-Patient - This profile defines the UK constraints and extensions on the International FHIR resource Patient. The Patient resource represents the patient involved in the provision of healthcare related services.
  • UKCore-Practitioner - This profile defines the UK constraints and extensions on the International FHIR resource Practitioner. The Practitioner resource represents the healthcare professional directly or indirectly involved in the provision of healthcare related services.
  • UKCore-RelatedPerson - This profile defines the UK constraints and extensions on the International FHIR resource RelatedPerson.
  • UKCore-Organization - This profile defines the UK constraints and extensions on the International FHIR resource Organization. The Organization resource represents the organisation that employs the healthcare professional.
  • UKCore-Location - This profile defines the UK constraints and extensions on the International FHIR resource Location. The Location resource provides information and details on the physical location and the services provided.
  • UKCore-Device - This profile defines the UK constraints and extensions on the International FHIR resource Device. It identifies an instance of a manufactured item that is used in the provision of healthcare without being substantially changed through that activity. The device may be a medical or non-medical device.

Outpatient Letter

The document bundle is a collection of FHIR Resource profiles combined to support Outpatient Letter (inpatient discharge summary) FHIR Documents.

The Bundle consists of the following FHIR Resource Profiles.

  • UKCore-Bundle - This profile defines the UK constraints and extensions on the International FHIR resource Bundle.
  • UKCore-Composition - This profile defines the UK constraints and extensions on the International FHIR resource Composition.

  • UKCore-Organization - This profile defines the UK constraints and extensions on the International FHIR resource Organization. The Organization resource represents the organisation that employs the healthcare professional.
  • UKCore-Location - This profile defines the UK constraints and extensions on the International FHIR resource Location. The Location resource provides information and details on the physical location and the services provided.

Note 1: This attachment Profile must not be used to send an unstructured Transfer of Care document, its purpose is to allow an attachment to be included within a structured Transfer of Care document.


Mental Health eDischarge

The document bundle is a collection of FHIR Resource profiles combined to support Mental Health eDischarge (inpatient discharge summary) FHIR Documents.

The Bundle consists of the following FHIR Resource Profiles.
