Introduction
This implementation guide is published by the Swedish eHealth Agency to support the use of R5 version of HL7® FHIR®© for representing data about the organizational structure and service offerings of healthcare and social service providers in a Swedish context.
The purpose of the implementation guide is to facilitate data exchange among government agencies, healthcare and social services providers, and other relevant stakeholders regarding information about healthcare and social service providers.
Background and Context
The need for this implementation guide arose as the Swedish eHealth Agency got tasked with developing the Swedish National Catalog of Organizations and Businesses, a common point of entry for publishing structured and standardized information about the content and organization of services within the sector.

Scope and Usage
The artifacts and interactions in this guide has been designed to meet two use cases - express organizational structures and express the service offerings of organizations, though it is encouraged to use or reference the artifacts in any use cases pertaining to organization and service data in a Swedish context.
Organizational structure
One use case of this guide is to express the organizational hierarchies of healthcare and social service providers. This is done by using two profiles of the Organization resource - one for expressing a legal entity (HVOOrganization) and the other one its units (HVOOrganizationalUnit). Furthermore the units can contain information about the businesses (HVOBusiness) they operate, which includes references to the location (HVOLocation) where they are operated.
The capability requirements pertaining to organizational structure data is expressed using the following Actor Definitions:
- Organization Repository: a server that can store organizational structure data.
- Organization Writer: a client that can write information about legal entities.
- Organizational Unit Hierarchy Writer: a client that can write information about hierarchies of organizational units.
Service offerings
Another use case of this guide is to express both procured and X services offered by healthcare and social service providers.
Boundaries and Relationships
The profiles on Organization reuse and build on the R4 Swedish base profiles from HL7 Sweden in order to ensure uniform profiling and terminology for common Swedish concepts. Note that the profiles in this guide do not technically inherit from the base profiles since they use the R4 version of FHIR and the Swedish national digital infrastructure is required to use R5.
