Patient and Demographics

These profiles cover patients and family history.

Profile Name	Description
Consent	The P-Care profile for recording a person's healthcare consent choices.
Patient	Demographics and other administrative information about an individual or animal receiving care or other health-related services.
FamilyMemberHistory	Significant health conditions for a person related to the patient relevant in the context of care for the patient.

Organizations and Locations

These profiles cover healthcare facility details such as organizations and locations.

Profile Name	Description
HealthcareService	The details of a healthcare service available at a location.
Location	Details and position information for a physical place where services are provided and resources and participants may be stored, found, contained, or accommodated.
Organization	A formally or informally recognized grouping of people or organizations formed for the purpose of achieving some form of collective action.

Providers and Roles

Profiles related to healthcare professionals and their roles within organizations.

Profile Name	Description
Practitioner	A person who is directly or indirectly involved in the provisioning of healthcare.
PractitionerRole	A specific set of Roles/Locations/specialties/services that a practitioner may perform at an organization for a period of time.

Clinical Conditions and Allergies

Profiles related to patient conditions, allergies, and clinical diagnoses.

Profile Name	Description
Allergy Intolerance	This profile supports the documentation of a risk of harmful or undesirable physiological response associated with exposure to a substance.
Condition	A clinical condition, problem, diagnosis, or other event, situation, issue, or clinical concept that has risen to a level of concern.
Procedure	An action that is or was performed on or for a patient.

Medications

Profiles related to medications, orders, and medication usage.

Profile Name	Description
Immunization	Describes the event of a patient being administered a vaccine or a record of an immunization as reported by a patient, a clinician or another party.
Medication	The P-Care profile for identifying and describing a medication.
MedicationRequest	An order or request for both supply of the medication and the instructions for administration of the medication to a patient.
MedicationStatement	The P-Care profile for recording a person's reported medication use.

Observations and Results

Profiles for various types of clinical observations, including lifestyle assessments and diagnostic results.

Observation Alcohol Use	The P-Care profile for recording the number of alcoholic drinks consumed per day.
Observation Lab Pathology	The P-Care profile for recording laboratory and pathology result observations.
Observation Radiology	The P-Care profile for recording radiology result observations, including textual, coded, and numeric component results.
Observation Social History	The P-Care profile for an observation about a person's social history and circumstances relevant to care.
Observation TobaccoUse	The P-Care profile for recording tobacco smoking status.
Observation	Measurements and simple assertions made about a patient, device or other subject.

Binary	A resource that represents the data of a single raw artifact as digital content accessible in its native format.
Bundle Document	The P-Care profile for exchanging a clinical document Bundle.
Bundle Patient Summary	The P-Care bundle profile for exchanging a Patient Summary.
Task	The P-Care profile for the Task that is part of a bundle.
Composition	The P-Care profile for the structure and clinical attestation of a healthcare document.
Composition Patient Summary	A patient summary reconciling PS-CA and PS-AB requirements. Shared structural constraints are mandatory; Alberta-only structural differences are reported as compatibility warnings
DiagnosticReport	The findings and interpretation of diagnostic tests performed on patients, groups of patients, devices, and locations, and/or specimens derived from these.
DocumentReference	The P-Care profile for identifying and accessing a clinical document.
ImagingStudy	Representation of the content produced in a DICOM imaging study.
QuestionnaireResponse	The P-Care profile for answers to a structured questionnaire.

Profiles supporting care planning, scheduling, communication, and clinical services.

Appointment	This profile supports the booking of a healthcare event among patient(s), practitioner(s), related person(s) and/or device(s) for a specific date/time
Communication	The P-Care profile for recording information that has been communicated.
Encounter	An interaction between a patient and healthcare provider(s) for the purpose of providing healthcare service(s) or assessing the health status of a patient.
ServiceRequest	A record of a request for service such as diagnostic investigations, treatments, or operations to be performed.
