Overview

Administrative data can be linked to clinical data to provide better context and improve the quality of this information. For example:

• An allergy recorded against a patient’s record can be linked to the clinician who identified this, together with their grade and specialty.
• An encounter record for an admission or outpatient consultation will refer to the organisation in which it took place, together with details of the location, and the clinician(s) who attended the patient.
• A request for diagnostics will refer to the requesting clinician, together with details of where the location it has been requested and where result should be returned to.
• A patient demographic record may refer to the GP practice to which they are registered.

Data Model - Organizations and Locations

The diagram below shows the typical relationship between NHS Wales primary and secondary care organizations and locations.

Organization

The Organization resource contains information about health and care organizations. Examples for Wales include Health Boards and NHS Trusts, Hospitals, Dental practices, GP practices and GP clusters, but could be any public, private or voluntary sector organization whose activities encompass the funding or provision of health care services.

Location

The Location resource contains information about a physical space where care is provided such as a room, ward or clinic, vaccination centre or pharmacy. It can also describe any location where resources and participants may be found, contained, or accommodated, such as a cupboard, locker or storeroom. Location resources contain detail about address and location, times of operation.

As shown above, locations are considered to be part of a managing organization. The main location extension extends the Organization resource to support the exchange of information on the organization's main location.

Data Model - Clinicians and Roles

The diagram below show how clinicians and their roles are modelled in FHIR.

Practitioner

The Practitioner resource contains detail about a clinician or other care provider, such as formal name, title, professional registration numbers and other identifiers e.g. health board locally assigned identifiers, NADEX etc.

PractitionerRole

The PractitionerRole resource contains information about the clinician’s role within an organisation such as grade or specialty.

The model above shows that a clinician may have more than one role. For example a consultant oncologist employed by Velindre NHS Trust may also be contracted to perform sessions within another organisation.
