Scope

This implementation guide is developed as part of Ontario Health's plan to facilitate the exchange of Medical Imaging (MI) data. Ontario MI Clinical Data Repository (miCDR) is designed to enable authorized users to view MI data held within the EHR.

The purpose of this implementation guide is to develop guidance and explore potential of using FHIR standard for MI data contribution and consumption. For data contribution, this implementation guide defines the structure and terminology required for the submission of medical imaging data. This information supports OCINet's data migration from the legacy DICS platform, and ongoing MI data submission from DIRs. The query component of the implementation guide will be used to support medical imaging data retrieval from the EHR to the provincial viewers and other point of service systems. The use of term "contribution" refers to the submission of medical imaging data to provincial EHR, while "submission" refers to the technical aspect of the contribution, which are used more when defining FHIR artefacts such as profiles.Similarly, the term "submit" and "query" are used when describing API functionality from a business context. and "create", "update", and "search" are used to describe the technical interactions for these functionalities

The current design of this implementation guide focuses on FHIR representation of MI reports, MI orders and image manifests for submission from OCINet, and also consumption by the EHR viewing channel (provincial viewers and other point of service systems). For submission, the FHIR profiles in this implementation are based on data availabilities at OCINet, and they should not be interpreted as the provincial FHIR model of medical imaging data. For data consumption, the profiles are based on the current design of Ontario EHR profiles that are based on provincial MI data model.

Direct contribution of medical imaging data from hospitals and ICHSCs is out of the scope of this implementation guide. It will be supported by future FHIR implementation guide and HL7 v2 specifications.
