Diabetes Use Case

Introduction


Objectives

The Virtual Ward has seven objectives, these can be done in any order:

Objective 1

Attends an appointment 3 weeks before annual review

  • Given I am a healthcare assistant involved in a patient's care,
  • When a patient is booked in for a diabetes annual review,
  • Then tests need to be performed and observations need to be recorded.

Objective 2

Patient completes form following HA appointment pre review

  • Given I am a patient with diabetes,
  • When an annual review has been booked and test results have been received,
  • Then I need to complete a form with my goals.

Objective 3

Attends annual review

  • Given I am a practice nurse involved in a patient's diabetes care,
  • When a patient is seen for a diabetes annual review,
  • Then I need to see their test results and submitted information around their diabetes care.
  • Then a care plan can be created to coordinate the patient's care.

Objective 4

Submits bp readings online

  • Given I am a clinician involved in the patient's diabetes care,
  • When a patient submits blood pressure readings using an online form,
  • Then I can adjust their medication dose accordingly.

Objective 5

Collects self management data

  • Given I am a patient with diabetes,
  • When I record blood sugar readings using a glucose sensor,
  • Then the data can be used to manage my diabetes care.

Objective 6

Submits information around concerns

  • Given I am a patient with diabetes,
  • When a review has been booked in with a clinician,
  • Then I need to complete a form listing my concerns and information around my diabetes care.

Objective 7

Attends an appointment with consultant diabetologist

  • Given I am a clinician involved in a patient's diabetes care,
  • When a patient is seen for a review,
  • Then I need to see their blood sugar readings.
  • Then I need to see their information that they submitted around diabetes care.
  • Then I need to create a plan for managing their diabetes care.
