Issue Assessment Notice Formal Notice of request for Assessment under The Care and Support (Discharge of Hospital Patients) Regulations 2014.

Note - most of the data required by the act will already be contained on the initial referral, and the Assessment Notice may be contained within the ReferralRequest. However, the API supports an initial referral without, followed by a separate Assessment Notice; for example for admission notifications where patient consent for a formal assessment may not yet have been sought

Issue Discharge Notice Formal Notice to the local authority of the proposed date of the patient’s discharge. This will generally be issued when the health activities required to safely discharge the patient are complete (or nearing complete) and therefore the trust is confident that the proposed discharge date is accurate and it is unlikely that the notice will need to be withdrawn and/or reissued PUT and

Withdraw Discharge Notice A discharge notice must be withdrawn where the NHS body considers that it is no longer safe to discharge the patient from hospital on the proposed discharge date.

Note - The NHS body does not need to withdraw the discharge notice if the delay to safe discharge is a because the local authority has not completed the assessment / put in place care and support to meet any eligible needs.

Share Case Note with Social Care Subsequent to referral, health workers may wish to share a note will the responsible local authority, for example and update on the outcome of an activity which may have a bearing on the social care needs. POST /ReferralService/v3/Communication Live
Update Predicted Discharge Date Inform the responsible local authority of a change in the predicted date of discharge for the patient. PUT /ReferralService/v3/Encounter Live
Update Patient's Location Inform the responsible local authority of a change in the ward/hospital that the patient is currently in. PUT /ReferralService/v3/Encounter Live
Update Safe for Discharge Status Inform the responsible local authority of a change in whether the patient is safe to dischrge or not. PUT /ReferralService/v3/Encounter Live
Confirm patient discharge Confirm to the responsible local authority that the patient has been discharged on clinical advice or with clinical consent. PUT /ReferralService/v3/Encounter Live
Notify Social Care that patient has died Inform the responsible local authority that the patient has died, and that the referral is therefore cancelled, and any open Assessment/Discharge Notices are withdrawn. PUT /ReferralService/v3/Encounter Live
Notify Social Care that patient has self-discharged Inform the responsible local authority that the patient has self-discharge, and that the referral is therefore cancelled, and any open Assessment/Discharge Notices are withdrawn. PUT /ReferralService/v3/Encounter Live
