Ontario Surgical Efficiency Reporting Information System Conceptual Data Model
The primary data model for the collection of Ontario Surgical Efficiency Reporting Information System resides in the main project documents that are referenced in the References section of this guide. The Conceptual Model presented below is tied directly to the HL7 FHIR interface specification and uses FHIR Resources for entities for the transmission of the SERIS data.
The model does not repeat the structure of the messages laid out in FHIR. Links to FHIR specs are included for entity definitions and field code values. In any difference of meaning between cited FHIR elements and this model, FHIR is the authoritative voice. This model relies on the following specifications:
- FHIR R4 (4.0.1) - The current official version of FHIR used at Ontario Health as of the time this implementation guide was published.
Conceptual Data Model ERDs and Metadata
Entity Relationship Diagram (ERD) follows. This include:
- a conceptual high-level ERD for solution data structures for the Release 1 solution, including metadata for each entity.

Appointment | The focal entity for an OR case. ties together the Procedure (with associated Practitioners) to the Patient |
Encounter | Entity that records the interactions between a patient and health care providers |
Procedure | Contains the information on the surgical operation to be performed (or has been performed) on a Patient |
MedicationAdministration | The information on the anaesthetic that was administered to the Patient for the purposes of performing the surgical procedure. |
Patient | The individual on whom the surgical procedure will be/has been performed. |
Practitioner | The individuals either performing the surgical operation or administering the anaesthetic. |
Observation | The entity that holds the American Society of Anesthesiologists physical status |
Location | A physical place where a surgical procedure can be performed. |
Organization | A formally recognized grouping or institution within the Ontario healthcare setting that is responsible for overseeing the locations where surgical procedures can be performed. |
Slot | A period of time on a schedule that may be available for booking OR Cases. |
Schedule | A container for slots of time that may be available for booking OR cases. |
PractitionerRole | The entity that holds speciality of a practitioner |
Task | Represents various business statuses of the OR case |

OR Schedule Conceptual Solution
The SERIS OR Schedule captures the planned start and stop times for each OR in use and any adjustments for room openings and closures throughout a planning horizon. Schedules shall identify time blocks for Surgeon(s) or Service(s) if applicable.
OR Case Conceptual Solution
A container for information about an operating room encounter where a patient is expected to receive one or more surgical procedure(s). The FHIR bundle is comprised of multiple FHIR Resources that support the capture of the Appointment and partial Encounter workflows.
