Scope
The Ontario eForms HL7® FHIR® Implementation Guide defines how standardized, structured electronic forms (eForms) are designed, rendered, and processed using the HL7 Structured Data Capture (SDC) framework.
This guide focuses on form rendering and behavior — specifically how FHIR-based forms are displayed, completed, and exchanged across Ontario’s digital health ecosystem. The eForms Implementation Guide applies to the following components:
- Definition and implementation of Ontario FHIR resource profiles and extensions within eForms Designer.
- Use of standardized value sets and code systems.
- Rendering and presentation of Questionnaires across various Referral Management Systems in Ontario.
- Integration with launch contexts and attachment handling.
Out of Scope
- Backend integration or business workflows unrelated to FHIR
- Non-FHIR or paper/PDF-based form tools
Applying the eForms HL7 FHIR IG
Ontario Health will be establishing the Ontario eForms HL7 FHIR IG as an interoperability specification under Ontario’s Digital Health Information Exchange (DHIEX) framework.
