Descripción del Arquetipo: Riesgo de respuesta fisiológica indeseable individual asociada con la exposición a una sustancia. Comentario: Las sustancias incluyen, entre otras: medicamentos administrados correctamente a dosis adecuadas, comida, material derivado de plantas o animales y otros.

Objetivo: Registrar la condición clínica de una propensión a una reacción adversa por exposición futura a una sustancia especificada, o clase de sustancia, basada en uno o varios eventos clínicos pasados atribuidos a la exposición a la sustancia o grupo de sustancias.
Uso: Se propone su uso para proporcionar un lugar único dentro de la historia de salud en el que almacenar la propensión del individuo para una posible reacción adversa futura tras una nueva exposición.
Dicha información se documentará desde las aplicaciones de uso habitual (HIS de cada hospital, eSIAP, y otros).
La información documentada en una aplicación viajará a la historia de salud que la remitirá al resto de aplicaciones, pudiendo crear efectos adversos que no existían para un paciente o modificando los ya documentados por otro profesional. La reacción adversa puede ser:
- Una reacción inmunomediada (Tipos I-IV; incluidas reacciones alérgicas e hipersensibilidades)
- Una reacción mediada no inmune, que incluye reacciones pseudo-alérgicas, efectos secundarios, e intolerancias. En la práctica clínica, la distinción entre las reacciones mediadas por mecanismos inmunológicos y las no inmunes es difícil. La identificación del tipo de reacción no es un sustituto de la gravedad o el riesgo de daño al paciente, que se expresa mejor por la manifestación en la práctica clínica.
El riesgo de una reacción adversa no debe registrarse sin identificar una sustancia o clase de sustancia propuesta. Cuando exista incertidumbre de que una sustancia específica sea la causa, esta incertidumbre puede resolverse utilizando el atributo 'Estado'.
Si hay varias sustancias posibles que pueden haber causado una reacción / manifestación, cada sustancia se debe registrar usando un ejemplo separado de este arquetipo de reacción adversa con el "estado" establecido en un estado inicial de "sospechado" para que se pueda verificar la reacción adversa. Uso Incorrecto: No debe usarse para registrar reacciones fisiológicas a agentes físicos, como calor, frío, luz solar, vibración, actividad de ejercicio, agentes infecciosos o contaminantes de alimentos.
No debe usarse para registrar fracasos de procesos clínicos, intervenciones o productos. Por ejemplo: uso anormal o consecuencias de errores cometidos por mala administración de un agente o sustancia; por dosis incorrectas; por mal etiquetado; daños o lesiones causadas por una intervención o procedimiento; sobredosis / envenenamientos, etc.
No se debe usar para registrar una reacción adversa cuando la sustancia se desconoce. Así, no se debe documentar “alergia medicamentosa no especificada” o “reacción adversa a sustancia desconocida”.
No debe usarse para registrar reacciones a transfusiones de productos sanguíneos. Tal y como propone openEHR y HL7 FHIR se creará un arquetipo específico.
No debe usarse para registrar terapias fallidas.
