Finnish PHR Spirometry Measurement Profile
The profile is based on the Finnish PHR Vital Signs Base profile UPDATED LINKKI KORJATTAVA. This profile alone does not describe all the elements needed and most of the elements are visible only in base profile. This profile describes changes to the base profile and the implementers must also familiarize themself with the base profile.
A full list of the extensions, constraints and a structured definition of this profile can be found in Simplifier. There are also different views of the profile (details, mappings, table, XML, JSON).
The Finnish PHR Spirometry Measurement profile constrains the Finnish PHR Vital Signs Base Profile as follows:
Mandatory elements for Finnish PHR Spirometry Measurement
Observation subject and performer has to reference to FinnishPatient.
Content | Name in structure | Description / Type of information |
---|---|---|
Code for spirometry measurement | code | Code describes the measurement mode that has been in use in the measurement. Codes SHALL always be from the Finnish PHR measurementMode ValueSet. Read more about the Finnish PHR measurementMode ValueSet. |
Optional elements for Finnish PHR Spirometry Measurement
Content | Name in structure | Description / Type of information |
---|---|---|
Measurement Phase | extension:phaseOfMeasurement | Phase of the spirometry study where measurement belongs to (for example: pre/post/baseline/control/challenge/recovery). In case of self-measured monitoring studies, it tells has the measurement been made before or after the medication. The code SHALL always be from the Finnish PHR phaseOfMeasurement ValueSet. Read more about the Finnish PHR phaseOfMeasurement ValueSet |
Period of they day when measurement has been performed | extension:dayPeriodOfMeasurement | Period of the day when the measurement has been performed (choices: morning/daytime/evening/nighttime). This is essential information in self-measured monitoring studies, where values are followed regularly mornings and evenings. The code SHALL always be from the Finnish PHR dayPeriodOfMeasurement ValueSet. Read more about the Finnish PHR dayPeriodOfMeasurement ValueSet |
Has the measurement been excluded from the results | extension:isMeasurementExcluded | Has the measurement been excluded from the results. User or application may exclude it for a bad quality/other reasons but clinician may still want to see it. It is important to store also excluded measurements because they may be excluded for wrong reasons or interprator may have a different criteria for excluding measurements. |
Prediction model for the measurement | extension:predictionModel | Name of the prediction model used in the study (for example "GLI, caucasian" or "Kainu"). |
Order (indices) of top breaths (inhale/exhale). | extension:breathTopOrder | Order (indices) of top breaths (inhale/exhale). Format: [index of best breath, 2nd best, 3rd best]. Typically measurement consists of multiple breaths, that are ordered according to selected spirometry standard recommendation. These indices are essential information picking the desired results from "Finnish PHR Spirometry Result" instances. Storing all breaths is necessary to be able to recalculate the results, if standard recommendation changes. |
Reproducibility values of top inhale/exhale breaths. | extension:breathReproducibility | Reproducibility values of top inhale/exhale breaths. Format: [reproducibility between best and 2nd best breath, reproducibility between best and 3rd best breath]. Reproducibility is an essential parameter to evaluate the quality of spirometry measurements. Importance of the reproducibility information raises in case of self-measured studies. |
Results resources belonging to this measurement | hasMember | References to resources related to measurement: Spirometry breath profile (Read more about the breath profile), spirometry result (representative) profile (Read more about the result profile), Finnish PHR height and weight profiles. |

Elements in the Spirometry Measurement profile filled by the Kanta PHR platform
Content | Name in structure | Description / Type of information |
---|---|---|
Application that created or updated the resource | extension: creatingApplication |
Is used to represent information about the application (id and name) that created or updated the resource. See the definition for the Finnish PHR Application Information extension. Kanta PHR platform will fill in this data automatically to each stored resource (if the application has entered some data in this extension, it will be overwitten). |
Narrative guide
Narrative SHALL contain following mandatory information taken from the elements of the resource instance:
- Code: <Observation.code.coding.display> ( <Observation.code.coding.code> )
- Time: <Observation.effectiveDateTime>
- Result: results are stored in the referenced Result resource
Narrative MAY contain following optional information taken from elemts of the resource:
- Measurement phase:< extension:phaseOfMeasurement.valueCodeableconcept.coding.display>(<Observation. extension:phaseOfMeasurement.valueCodeableconcept.coding.code>)
- Day period: < extension:dayPediodOfMeasurement.valueCodeableconcept.coding.display> ( < extension:dayPediodOfMeasurement.valueCodeableconcept.coding.code)>
- Is measurement excluded: < extension:isMeasurementExcluded.valueBoolean >
- Prediction model: < extension:predictionModel.valueString >
- User's notes: <Observation.note.text>
Narrative example:
- Code: FEV6 mode (fev6-mode)
- Time: 2020-04-27 12:00
- Result: results are stored in the referenced Result resource
- Measurement phase: Pre-bronchodilation (qualifier value) (255274008)
- Day period: Morning (qualifier value) (73775008)
- Is measurement excluded: yes
- Prediction model: Koillinen 1998 (6-17)
- User's notes: User's own notes related to measurement (for example, coughing before measurement)
If element is missing from resource, it shall not be mentioned in narrative.
