Care Interventions

Care interventions - i.e. such interventions at patients as carried out on a daily routine by nurses and care personal.

It involves the usage of the following medico FHIR profiles (each one may have multiple extensions). The setup of these profile largely follows [FHIR's workflow model] (https://www.hl7.org/fhir/workflow.html):

  • ActivityDefinition_ActivityCareInterventionPlan_1

    This is master data: An instance of this profile represents an activity definition for one specific care intervention which is specified by code property and refers to a TopicGroup via topic property. This ActivityDefinition is referenced by CarePlane-CareInterventionPlan_1. It references ObservationDefinition_CareInterventionExpectedResults_1. Thereby it defines, what kind of observations are expected as the outcome of a concrete CarePlan instantiated for an encounter.

  • ObservationDefinition_CareInterventionExpectedResults_1

    Defines what kind of observations are expected from a careplan. This is done by providing the matching code, e.g. LOINC codes. Those code could represent a blood pressure - thereby specifying that a careplan - with the underlying ActivityDefinition - should lead to observations providing a blood-pressure with that given code. if later an observation is created - based on the code provided with ObservationDefinition - there is no tracable connection between the observation and the careplan-resource.

  • CarePlan-CareInterventionPlan-1

    And instance of this resource type represents a time-framed care-intervention of a specific type, for a specific encounter - and a specific interval. Therefor it also has exactly one activity - and one nested "detail" within that acitity.

  • Servicerequest_Careinterventionrequest_1

    The procedure request represents a plan for a specific time to perform a care intervention. It carries the code of action to be executed. In its basedOn field it carries the reference to the initiating CareInterventionPlan.

  • Procedure_CareIntervention_1

    The procedure "implements" its counter-part, the procedure request. It exists when the request is carried out and/or documented. Its either uses basedOn field to reference to the initiating CareInterventionPlan or the definition field to reference the activity definition in case it was an unplanned care intervention.

  • Provenance-CareIntervention-1

    Up to 5 nurses or male nurses can participate in execeting care intervention procedure. If confirmaation of them carrying out that action is required - that the confirmation is expressed using this provenance resource.

  • Media-MetaIconForCareInterventionGroup-1

    An instance of this group represents an icon which is usually used for a Care intervention plan in the user interface. The icon is directly connected to the "CareInterventionGroup". This group is provided through the category slice named "CareInterventionGroup" of the CareInterventionPlan resource. Within that slice use that category's code - to retrieve this media ressource instance. if e.g. the code is "1234", prefix this code as stated within the media-profile's id field to retrieve it.
