Handling observations
This specification uses the Profile: CareConnect-GPC-Organization-1 profile for clinical, and social history observations.
The information below describes the sort of information that is expected to flow.
Social history observations
These types of observations will only contain a code - typically SNOMED, and will not contain a value. For example: pregnancy status, or to indicate that an Ambulatory blood pressure recording has been declined.
There are only a few scenarios where this will be the case, and the Mapping PRSB headings to FHIR page provides examples of these.
Vital signs observations
The Community Pharmacy Standard v3 stipulates the following items that could be sent.

While the provider should make every effort to ensure they are providing accurate information, there are certain conventions that are preferred within GP practice which are shown hin the table below.

Reference ranges and interpretation
Providers may supply a reference range along with the associated value - for example:

Self-reported measurements
As of Wednesday, 22nd November 2023, Section 4.18 of the NHS England Pharmamcy Contraception Service specification states the following:
4.18 For COC, a supply will require BMI and a blood pressure measurement to be taken in line with NICE guideline 136. A person accessing the service may also offer their own weight, height and blood pressure measurements. Any self-reported measurements that the pharmacist has deemed clinically suitable, will need to be recorded as such.

Care should be taken when recording these elements to ensure that accurate information is both being stored in the Pharmacy Record, and in the message that flows back to the GP - including correct SNOMED code usage.
How to send an ABPM report
It is possible that reports can be generated as an attachment - for example, a PDF or text file.
In those instances, Update Record will allow an attachment to be provided via the FHIR Observation resource, using the valueAttachment
element.
Consumers: Please ensure the document title is visible within the list of documents or other attachments in the patient's history.
