Record Management Approach

Within the PCR, the identifiers and demographic information received from different contributors are stored as received as distinct source records (member records).

Matching algorithms, with supporting business processes, are used to link the member records received to distinct patients (entities). Each entity is assigned a unique PCR managed Enterprise Client Identifier (ECID).

When a contributor updates a given member record (e.g. merge, unmerge, addition of missing HCN), the update may impact information that changes which entity (patient record) the member record is linked to. This, in turn, will change the ECID assigned to the member record.

Consumer systems may query the PCR to receive either: the full list of identifiers associated with a patient identifier, the most current demographic information associated with an enterprise client identifier (the ‘Golden’ record), or query using demographic information to search for patients.

The logical model for client data in the PCR is illustrated below.

Source C March 2015 Margret Jack, now married, provides her married name to a corporate model hospital. The corresponding hospital record is contributed to the PCR by the source, and the PCR ultimately links the hospital record to the same entity as the RCB & previous hospital record based on a positive matching score for legal name, address, and HCN.
Golden Record March 2015 and beyond When querying the PCR for Margret Jack or Peggy-Jack Ryan, PCR leverages the historical records (member records) to provide a candidate match for Peggy-Jack Ryan. The candidate result will include summary level ‘golden record’ demographics per the latest contributions at the attribute level (across all PCR sources), along with the aggregate list of active identifiers for the patient.

Patient Data

The following table summarizes the patient identifier types and demographic data currently collected within the PCR. The amount of patient information returned from the PCR in response to a query depends on the query operation used. Refer to the Profiles and Operations section for further guidance.

Note that:

  • Multiple instances of any given patient identifier may exist in a query response

  • Active vs. inactive patient identifiers are returned depending on the operation used

  • Some elements may not be populated in a given query response, based on the data density for the patient

PCR Enterprise Client Identifier (ECID) ID used to uniquely identify a patient in the Provincial Client Registry (PCR). The PCR engine uses a probabilistic matching algorithm to link together an individual’s demographic records from multiple sources under a single identifier known as the Enterprise Client Identifier (ECID), and thus can provide identity cross-reference services in support of the Electronic Health Record (EHR). Corporate IDs (CORPID) Organizational level corporate identifier that is used to uniquely identify a patient within a given source system and that is used to link multiple patient identifiers within the same source. An example is a corporate model hospital that consists of multiple sites and corresponding Medical Record Number (MRN) banks, which issues a separate MRN for the patient’s visit to each site, but manages all of the patient’s MRNs under a single Corporate Identifier (CORPID) and master set of demographics.
