1.3. Scope

This implementation guide is developed as part of Ontario Health's plan to facilitate the exchange of medical imaging (MI) data through Ontario MI Clinical Data Repository (miCDR). miCDR is a provincial solution designed to enable authorized users to contribute and consume MI data held within the EHR.

The purpose of this implementation guide is to develop guidance and explore the potential of using the FHIR standard for medical imaging (MI) data submission and consumption. This aligns with the provincial DHIEX Framework, as outlined in O. Reg. 329/04, which identifies FHIR as the primary healthcare interoperability standard for health data exchange in Ontario. The submission specified in this implementation is intended to support direct contribution of medical imaging data from hospitals and ICHSCs. The query component of the implementation guide will support medical imaging data retrieval for provincial viewers and other point-of-service systems.

Througout the implementation guide, the use of term "contribution" refers to the submission of medical imaging data to provincial EHR, while "submission" refers to the FHIR submission of the contribution when defining FHIR artefacts such as profiles. The term "Consumption" refers to the query of MI data from EHR, specifically miCDR, and "query" refers to the FHIR search query for specific MI use cases. Similarly, the term "submit" and "query" are used when describing API functionality from a business context and "create", "update", and "search" are used to describe the technical interactions for these functionalities


1.3.1. Applying MI FHIR IG

Ontario Health will be establishing the Ontario Medical Imaging HL7® FHIR® Implementation Guide-v1.0.0-draft1 as an interoperability specification under Ontario’s Digital Health Information Exchange (DHIEX) framework.

The following information is provided in accordance with O. Reg. 329/04 under Personal Health Information Protection Act (PHIPA), 2004, section 28 Application of Specifications.

Requirement Details
Date on which the specification becomes effective (Note:  This is the date by which all applicable HICs are required to be compliantly contributing and/or consuming the approved interoperability specification. OH will work with the applicable HICs to establish the implementation plan in order to meet the effective date.) N/A - Balloted draft
Name, description, or class of Health Information Custodians that must select, develop, or use digital health assets that comply with the specification: Health Information Custodians who contribute data to miCDR:
- Hospitals as per the Public Hospitals Act, R.R.O. 1990, Reg. 965.
- Integrated Community Health Services Centres (ICHSC) within the meaning of the Integrated Community Health Services Centres Act, 2023.

Health Information Custodians who view MI FHIR IG data:
- All HICs, as per Section 3 of PHIPA compliance, as per Ontario Health agreements, to access or integrate with the EHR
Types of digital health assets to which the specification applies Digital health assets used by Health Information Custodians for the above purpose:
- Electronic Medical Record System (EMR) and Radiology Information Systems (RIS)
- Point of service systems and viewers that queries and retrieves imaging data from miCDR (e.g. provincial clinical and patient viewers, current OH image viewer and future enterprise imaging viewer, provincial and regional Foreign Exam Management solutions (FEM)
Circumstances, if any, when a Health Information Custodian may be exempted from the requirement to select, develop or use digital health assets that comply with the specification: Contributors planning to contribute medical imaging data using the future Ontario Medical Imaging HL7 V2 Implementation Guide 1.0.0.
