Background

The eForms project sets out to build a solution with the ambitious vision to have all healthcare forms to always be up-to-date for all points of service and to be pre-populated where possible and delivered electronically.

The eforms solution will enable physicians to quickly and accurately input information electronically that can be automatically? mapped to a health data standard and exchanged with other systems for recipients for further use or transform into other purposes in their workflows.

What is the eForms solution and the technologies and adopted standards?

The eForms solution has different components with different owners and contributors. At the start, there is a form creation tool (a vendor solution) that enables form creators to build and customize forms to meet their business needs and requirements. The forms have standardized elements to enable mapping. The forms are mapped to a core data set that is based on FHIR standard, SNOMED CT terminology and LOINC codes. All of these are maintained in a Provincial Forms Repository. The users of forms access this repository via a viewer or an integration with their EMR system. Certain data elements will be pre-populated from other data sources and registries to minimize the need for manual data entry or selection. From there, the forms would be distributed via the CDX solution to specified connected systems such as a recipient physician’s EMR, a hospital’s clinical information system or a secure destination. There is a governance, development, quality control, publishing and administration process for this solution.
