Additional Guidance
Where the patient’s NHS Number is available, the associated Bundle SHALL NOT include a Patient resource. In this scenario, other resources in the Bundle SHALL identify the patient using subject.identifier populated with the patient’s NHS Number, for example:
"subject": { "identifier": { "system": "https://fhir.nhs.uk/Id/nhs-number", "value": "9727710638" } },
Support for scenarios where the patient’s NHS Number is not available will be documented in a future release of this implementation guide.
identifier
The Patient.identifier element should contain the patient's NHS Number where available.
For the NHS Number identifier system, see the relevant UK NamingSystems guidance.
Where an NHS Number is unavailable, a local patient identifier may be used in accordance with the applicable identifier guidance.
Validated NHS Number unavailable
Where a validated NHS Number is unavailable, the associated Bundle should include a Patient resource containing supporting demographic information and one or more patient identifiers that are not NHS Numbers.
An unvalidated NHS Number Should be treated as unavailable and Should not be included as an NHS Number identifier.
Other patient-linked resources within the Bundle Should reference the Patient resource using subject.reference.
For example:
"subject": { "reference": "urn:uuid:a4c2e1d3-5678-4321-abcd-ef0123456789" }
Where a validated NHS Number is unavailable, Patient.identifier Should contain one or more patient identifiers that are not NHS Numbers.
Each identifier Should contain:
identifier.systemidentifying the namespace for the patient identifier.identifier.valuecontaining the patient identifier within that namespace.
The identifier may be a local patient identifier assigned by the sending system or middleware. The identifier.system Should identify the namespace under which the identifier was assigned.
For example:
"identifier": [ { "system": "https://example.nhs.uk/patient-identifiers/local", "value": "11111111-2222-3333-4444-555555555555" } ]
