This Implementation Guide supports the project One Comprehensive Patient Plan for Chronic Disease. The project is part of the 9th semester of the Masters degree in Biomedical Engineering and Informatics at Aalborg University, Denmark.

The purpose of the artifact is to store and standardize data in the form of the general practitioner's records regarding the patient, so that the records can be saved as a complete patient plan, which contains one or more of the listed chronic diseases. For the artifact, technical interoperability is achieved by standardizing the registrations in the form of profiles made in HL7 FHIR. The profiles ensure the semantic understanding of the registrations through terminology bindings with the standards SNOMED CT and LOINC, so that general practitioners use the same terminology.

The project contains five profiles which have been modified to suit the Danish practioner. By implementing the profiles, the data from one patient plan will be standardized and ready-to-use in other patient plans for the same patient.
