Many organizations across the province provide health care services and contribute patient health information to Ontario’s Electronic Health Record (EHR) assets. These organizations will typically maintain a list of patients with identifiers and corresponding demographic information within their local systems to support local needs. The role of the Provincial Client Registry (PCR) is to facilitate accurate identification of patients (clients) across the different points where they receive care and to associate patient health information collected in these different places with the right patient as it is received by Ontario’s EHR services.
The PCR aggregates demographic data and identifiers from the Ontario Registered Persons Database (RPDB) and participating healthcare organizations across the province to provide a comprehensive source of patient identity and demographic information. The identifiers and cross references maintained in the PCR are then used by other EHR services to associate health information with the appropriate patient and to enable healthcare provider access to a person’s complete health record. Identifier cross references and demographic data stored in the PCR are also made available to authorized health care providers through query service endpoints.
