### Use Case 2 Purpose This storyboard illustrates how a referral source makes a referral without the use of referral service. This storyboard specifically illustrates a referral from a hospital to a local community mental health & addictions organization (CMH&A Organization). Note: A referral service is a system that acts as an intermediary between referral source and referral destination. This system has the ability to either perform resource matching or redirect referrals based on business rules. It may have the ability to check a referral prior to passing it on (e.g. to confirm if all required information is present, check eligibility, etc.). Additionally, the referral service can also act as a referral repository for storing referral information for retrieval. Triggers: Upon consultation with patient, hospital case manager determines that patient needs referral to Local CHM&A Organization. Assumptions: * Hospital has capability of sending electronic referral * Local CMH&A Organization has capability of receiving electronic referral Story Event: Adam Everyman, a 46 -year-old male was admitted to hospital via emergency after being brought into emergency by the police. He had been found wandering in traffic and putting himself in danger. He was admitted to Ontario General Hospital (OGH) for assessment and stabilization of his mental illness (schizophrenia). During Adam’s hospital stay Helen Helper, the social worker, discovers from Adam’s family doctor that Adam has not been attending appointments regularly and has not been compliant with medications. To facilitate discharge, Adam is referred to the OGH Short-term Case Management program to plan for more intensive follow-up. The case manager meets with Adam and refers him to the localCMH&A Organization for the Mental Health & Addictions Case Management program. Flow of Events: 1. Patient is admitted in hospital through emergency department 2. Patient is referred to hospital’s short-term case management program 3. Case manager meets with patient and determines referral is needed 4. Case manager refers patient to the Local CMH&A Organization case management program 5. Local CMH&A Organization case manager determines that patient is eligible for assessment 6. Local CMH&A Organization case manager completes assessment and accepts patient to the mental health case management program 7. Patient begins to receive service from Local CMH&A Organization Alternative Flows 8. Local CMH&A Organization declines referral as important data is missing 9. Local CMH&A Organization determines that patient is not eligible for assessment and declines the referral
