### Use Case 1 Referral Sent Through a Referral Service Purpose: This storyboard illustrates a scenario when a referral source sends referral through a referral service. In particular this storyboard shows a referral from hospital to rehabilitation center. Note: This storyboard does not illustrate the complete cycle of a referral. Please refer to other use cases for the other interactions. Note: A referral service is a system that acts as an intermediary between referral source and referral destination. This system has the ability to either perform resource matching or redirect referrals based on business rules. It may have the ability to check a referral prior to passing it on (e.g. to confirm if all required information is present, check eligibility, etc.). Additionally, the referral service can also act as a referral repository for storing referral information for retrieval. Triggers: Patient stabilizes and no longer requires acute care services Assumptions Assumptions: * Hospital has capability to send electronic referral * Rehabilitation centres have capability to receive electronic referral * Referral service has the ability to determine which rehabilitation centres the referral can be forwarded to * Referral service has the ability to allow referral source to select referral destination(s) Story Event: Adam Everyman, 63 year old married man, known to have high blood pressure, has been hospitalized for two weeks following a mild stroke episode. Dr. Aaron Attend, Adam’s attending physician, decides it’s time to transfer Adam to a rehabilitation center as his condition has stabilized and blood pressure has been taken under control. Mary Discharger, the discharge planner, submits a referral through a referral service which sends out the referral to multiple rehabilitation centers (Rehabilitation Centers 1, 2, & 3). Mary receives responses from 3 rehabilitation centers indicating they have all the information needed and are able to proceed with determining whether Adam can be accepted in their facilities. After the rehabilitation centers have completed their review, Mary receives notifications informing her of their ability to admit or not.Rehabilitation Centers 1 and 3 indicated that they are willing to admit Adam, while Rehabilitation Center 2 declined as they can not accommodate Adam’s medical. Flow of Events: 1. Patient admitted to hospital 2. Patient stabilizes and attending doctor decides it’s time to transfer patient to a rehabilitation center 3. Hospital submits referral through a referral service 4. Referral service sends out referral to multiple rehabilitation centers 5. Rehab centers respond indicating they have all information required to proceed 6. After reviewing the referral, Rehab 1 determines that they are able to admit patient and notifies hospital 7. After reviewing the referral, Rehab 2 determines that they are unable to accept the patient as they are unable to accommodate patient’s medical needs 8. After reviewing the referral, Rehab 3 determines that they are able to admit patient and notifies hospital Alternate Flows: 9. Rehab centers rejects referral as referral does not meet rehabilitation criteria 10. Rehab centers accept but on waitlist (if on waitlist, notify referrer of waitlist status with timelines) 11. Rehab centers accept conditionally (based on ability to meet admit criteria post-op, e.g. must be partially weight bearing post-op)
